In accordance with the provisions of Article 3, Paragraph 3 of the public fund receiving account Registration Act and Article 5 of the Ordinance for Enforcement of the Act, please refer to the following for the list of items (Registration of public money receiving account Information List) to be recorded in the register of public benefit payment accounts.

Information provided to the public authorities providing the benefit

(I) Name of financial institution

(ii) Name of store (name of head office, branch office, local office, etc.)

(iii) Type of deposit / savings (ordinary / checking)

(iv) Account number (in the case of Japan Post Bank, the symbol number)

(5) Name in Japanese

Information recorded along with account information

(1) Name (and name in Japanese)

② Address

③ Date of birth

④ Personal Identification Number

(e) Contact information (phone number, email address, etc.)

(vi) Name of the institution that accepted the application for registration, etc. (including registration of change of account, amendment, correction, and deletion of registered information, and consent to the provision of account information to Digital Agency in public authorities)

(vii) Date of application for registration, etc.

(viii) Date of processing (date recorded in the public benefit payment account registry)
